Enrolling:

1. Go to this link:  iaff587.medikeeper.com. 
2. Scroll to the bottom of the box and click on the grey box that says, “Or click here to register”. 
3. Enter your name, birthday and zip code.  (If you make an error while doing this, please contact us at 305-425-1938 so we can verify the information we have for you.) 
4. Once you have enrolled in that system, you can start to earn points by filling out the Health Questionnaire and participating in the various Challenges.

Enrolled, but Locked Out: 

Please email benefits@healthtrustmaff.org so we can verify your information and include the following:

• Your full name
• Your Login/Username
• Date of Birth
• Phone number to contact you

Members Interested in Joining a Challenge After It Has Started:

Please note: First you must enroll in Medikeeper (if not enrolled yet, see directions on how to do so above).

There are 2 ways to request access to this challenge:

1. Contacting Medikeeper Adminstrator.
   1. Log in to :  iaff587.medikeeper.com
   2. Click on “Wellness Challenges”
   3. “View Challenges”
   4. Click on “Step it Up” - if you can see the challenge, try to sign up again.  (If you are unable to sign up, there is a “Contact Administrator” button on the page.  Please click it and request to sign up.  You should send an email to the administrator that states you want to join late. 

1. Send an email to benefits@healthtrustmaff.org with the following information so we can request you get added to the challenge.

• Your full name
• Your Login/Username
• Date of Birth
• Phone number to contact you

NEW IN 2023!

EARN REWARDS FOR A PREVENTIVE SCREENING OR ANNUAL PHYSICAL

Make an appointment with your provider for your free annual wellness physical or assorted preventive care visits and earn 50 Ignite Wellness points.

The 50 points can be turned in to $50 worth of gift certificates as a reward for keeping healthy.

(Available to members and spouses/domestic partners enrolled in the L587 Health Insurance Trust)

To earn the points, you must first be signed up for the Ignite Wellness Program.

Once you are signed up for the Ignite Wellness Program:

1. Make an appointment between now and 12/31/2023 for any of the following screenings:
   • Annual Wellness Physical
   • Mammogram
   • Colonoscopy
   • Prostate
   • Well Women Visit
   • Skin Exam
   • Dental Exam
   • Vision Exam
2. Take the preventative screening form with you to the appointment.  To download and print the form, click this link: 2023 Preventative Screening Physician Form  or the image above.
3. You and your doctor will fill in, sign, and date the form.
4. Log into your wellness portal iaff587.medikeeper.com.
5. Click on the "Incentives and Rewards" tile.
6. Click on the "upload box" and upload the completed form.
7. Once your form is verified by Medikeeper you will be rewarded the 50 points.

Only one preventive screening is eligible for the 50 points, but we encourage you to go to all the appropriate screenings in 2023.

Fruits & Veggies Challenge

The enrollment period is: 
Monday, April 17th until Monday, May 1st 

The challenge period will be:
Monday, May 1st to Sunday, May 21st

Eat at least the recommended 5 daily servings of fruits & vegetables during the challenge period & earn 25 points.

Here is how to participate:

1. Enroll* in the challenge on the wellness portal at iaff587.medikeeper.com
    

• On the homepage, click on "Wellness Challenges"
• Click on "View Challenges"
• Click on the "Fruits & Veggies Challenge" located underneath "Available Community Challenges"
• Click on "Enroll in Challenge" located on the right side
   

2. Track your fruit & veggie consumption throughout the challenge
