CONTACT LENS ALLOWANCE FOR DEPENDENTS UNDER 19. The Board of Trustees of the IAFF Local 587 Health Insurance Trust Fund (Trust) approved increasing the contact lens benefit for children aged 19 years and under by removing the annual maximum allowance on contact lenses for the 2020 & 20201 Plan year. This means the coverage will be expanded for dependent children on the plan from a $150 reimbursement per year to covering a year’s worth of contact lenses costs. This mirrors what the City of Miami offered in 2018.
This increase in benefit will terminate on December 31st, 2020.
ADMINISTRATION OF THIS BENEFIT/REIMBURSEMENT REQUESTS: Unfortunately, Cigna has advised that it cannot administer the approved benefit through their system. For costs over $200 for contacts for children on the plan aged 19 and younger, the member will be reimbursed directly from the Trust.
The member is required to fill out a reimbursement form and provide appropriate documentation to be reimbursed. Reimbursement shall be limited to reasonable and customary costs of children’s contacts per each set purchased.
Reimbursement requests for expenses during January 1, 2020 - December 31, 2020 may be turned in at any time prior to March 31, 2021.
Reimbursement requests shall be submitted to the Benefits Administrator by mailing a reimbursement form and required back up information to:
